Health Insurance Marketplace
Get your 1095-A
If you bought health insurance through the Marketplace (Obamacare)
If this applies to you: We cannot file your federal return without this form.
How to get it:
- Log in to your account at HealthCare.gov.
- Select your prior year application (the year you're filing taxes for).
- Click "Tax Forms" from the left-hand menu.
- Download and print the 1095-A shown for that year.
Lost your login? Call the Marketplace directly at (800) 318-2596 to have them mail you a replacement.

IRS Identity Protection
Retrieve your 6-digit IP PIN
If the IRS has issued you a 6-digit Identity Protection PIN
If this applies to you: Your e-file will be rejected without it.
How to get it:
- Go to IRS.gov/IPPIN and click "Get an IP PIN".
- Sign in to your ID.me account to view your current 6-digit code.
- If you can't log in, call the IRS at (800) 908-4490 to have a replacement letter mailed.

Indiana Unemployment
Get your 1099-G (Indiana)
If you received unemployment benefits from Indiana during the tax year
If this applies to you: We must include this on your return.
How to get it:
- Log in to your Uplink account on the Indiana Department of Workforce Development website.
- Look under "Correspondence" to find your 1099-G document.
