Get Your 1095-A (Marketplace Insurance)
Important: If anyone on your return had health insurance through the Marketplace (Obamacare), we cannot file your taxes without this form. Click here for how to obtain your 1095-A online via Healthcare.gov's detailed instructions.
How to get your form online:
Log in to your account at HealthCare.gov.
Select your prior year application (the year for which you are currently filing taxes).
Click "Tax Forms" from the left-hand menu.
Download and print the 1095-A shown for that year.
Lost your login? Call the Marketplace directly at 1-800-318-2596 to have them mail you a replacement.
Retrieve Your 6-Digit IRS IP PIN
Note: If the IRS issued you an Identity Protection PIN, your return will be rejected by the IRS without it.
How to retrieve it:
Go to IRS.gov/IPPIN and click "Get an IP PIN".
Sign in to your ID.me account to view your current 6-digit code.
If you cannot log in, call the IRS at 1-800-908-4490 to have a replacement letter mailed to you.
Get Indiana Unemployment Form
You must provide your 1099-G if you received unemployment benefits during the tax year.
How to get it:
Log in to your uplink account on the Indiana Department of Workforce Development website.
Look under "Correspondence" to find your 1099-G document.
