1095-A

If anyone on your return had a Marketplace plan in 2022, you will get Form 1095-A, Health Insurance Marketplace® Statement, by mail no later than mid-February. It may be available in your HealthCare.gov account as soon as mid-January.
IMPORTANT: You must have your 1095-A before you file or your return will be rejected.  Don’t file your taxes until you have an accurate 1095-A.

Note:  You can have them mail you a new 1095-A if you lost it by calling 1-800-318-2596.

How to find your 1095-A online

Note: Your 1095-A may be available in your HealthCare.gov account as early as mid-January, or as late as February 1.
Log in to your HealthCare.gov account at https://www.healthcare.gov/login
Under "Your Existing Applications," select your 2022 application — not your 2023 application.
Select “Tax Forms” from the menu on the left.
Download all 1095-As shown on the screen.

ID Theft PIN

IRS Has instructions to obtain your ID Theft PIN at the below URL or you can call them at 800-908-4490.

Another way to obtain the number, if you have an IRS account, is to sign into your account.  Once signed in, scroll to the bottom and under notices and letters you should see a letter saying "We assigned you an IP PIN".

Indiana Unemployment

You need to signin to your uplink account to obtain your 1099-G for unemployment. The document is under correspondence.  For more detailed instructions click here.

    Personal Organizer

    The request below is no longer valid.  We will reopen this option January 2025. 

    To request an organizer be sent to you via secure portal upload, please enter your name in the designated section (taxpayer or spouse; either works). There are two email fields for your convenience. The portal will be created with the first email that matches our records. Note: If the spouse's email is entered first, the portal will be setup with the spouse's email.

    If you're uncertain about the email on record, fill in both email spaces (only one is required).  It's crucial to ensure that the email provided matches our records; otherwise, you will not receive an organizer within 24-48 hours. In the event that you do not receive the organizer, feel free to visit our office to request the personalized version until January 25th.  After January 25th, you'll use our blank profile sheet as you've done before.  This is found on our About Us page as well as in our office - exactly as in past years.  If you wish to receive the personalized organizer in future years, simply mark back of your profile sheet to indicate you'd prefer an electronic organizer next year.
